The Personal Data Document provides the college with basic information concerning the sharing of records and academic, health, and billing information, as well as emergency contact information. It is mandatory that you complete this document.

The RAVE Broadcast System is used to notify all students, faculty, and staff of campus wide emergencies and notifications. This includes, but is not limited to campus closings, inclement weather, active shooter, campus wide announcements, and location changes for campus wide events. The RAVE system will send a text and/or email with information to all users. The information shared through the texts and emails will be brief and informative.
All students are included in the RAVE system. If you would like to be taken off the RAVE list contact the Student Programs and Services Office. Once classes begin, we will do a test RAVE broadcast to ensure everyone receives the text and email.
Your grades and billing statement cannot be shared until the following is checked.

As a Eureka College student, you are responsible for fulfilling your financial obligations to the College. Any expenses incurred by EC as a result of my delinquencies in repayment of any funds or services provided by EC and/or by the financial aid programs shall be my responsibly and shall include collection costs and reasonable attorney fees necessary to recover any such delinquencies as set forth in the EC policies.
Eureka College is required by the Internal Revenue Service to provide every student with a 1098-T tuition statement. These forms are available to students/Alumni in late January of each year via the student portal. The 1098-T provides the student/parent with information regarding qualified tuition and related expenses that will assist with individual tax preparation.
Required if student is under the age of 21. Please use spouse, relative, or guardian if parent information does not apply.
